How To Make A Referral

The Insurer
  • Confirm referral to Jack or Edelweiss with employee's representative.
  • Send medical report confirming functional limitations. Include date of medical eligibility & QIW determination, U/C occupation, AVG Wage, TTD/VRMA rate.

Attorney

  • Confirm selection of Jack or Edelweiss Geary with the Insurer.
  • Request employee call Jack or Edelweiss to schedule an appointment.

Physician

  • If patient/employee is represented, tell the employee to have the attorney make the referral to Geary & Associates.
  • If patient/employee is not represented, tell employee to contact the employer or the claims administrator and request Geary & Associates.

When Authorized Geary & Associates will:

  • Contact the QIW within 48 hours of the authorized referral.
  • Schedule the initial interview within five days of the referral.
  • Complete Vocational Evaluation & Initial Assessment Report within 14 days of the initial interview.
